Initial Appointment

On the day of your appointment, you will be asked to fill out some information, including your health history. We ask that you bring a list of your medications, the dosages, and what you are taking them for. Please alert our staff, prior to your appointment, if you have a heart condition or any artificial joints, as these may require that you pre-medicate with antibiotics.

Important: All patients under the age of 18 must be accompanied by a parent or guardian, at the initial appointment.
