On the day of your appointment, you will be asked to fill out our patient registration forms, including your health history. Your visit will consist of a consultation explaining your diagnosis and treatment options. If treatment has been scheduled at time of consultation, we will provide treatment that day. If you are scheduled for a consultation only, we will require a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any X-rays, if applicable.
- A list of medications you are presently taking, the dosage, and what you are taking them for.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or legal guardian must accompany patients under 18 years of age, to their initial visit.
Please alert our office prior to your appointment if you have a heart condition or any artificial joints, as these may require you to pre-medicate with antibiotics. We also need to be alerted if you have a medical condition that may be of concern prior to surgery, such as, diabetes or high blood pressure, or if you are taking any of the following: heart medications, aspirin, or anticoagulant therapy.
If your previous dentist has taken recent x-rays (within 6 months), you may request that they forward them to our office. If additional films are necessary, they can be taken at our facility.
